 Zepbound® (tirzepatide) is a once-weekly, subcutaneous injection approved for chronic weight management in adults with obesity (BMI ≥30) or overweight (BMI ≥27) who have at least one weight-related condition such as type 2 diabetes, hypertension, or dyslipidemia. It is used in combination with a reduced-calorie diet and increased physical activity.The 2.5 mg/0.5 mL injection is the starting dose in the Zepbound titration schedule. This introductory dose helps patients begin treatment gradually to minimize gastrointestinal side effects before escalating to higher, therapeutic doses.

How It Works:
Tirzepatide is a dual GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptor agonist. By activating both pathways, it helps – Reduce appetite, Enhance insulin sensitivity, Delay gastric emptying, Decrease food intake. These combined actions support meaningful and sustained weight loss while improving metabolic health.

Dosing and Administration:

  • Dose: 2.5 mg once weekly 
  • Route: Subcutaneous injection 
  • Sites: Abdomen, thigh, or upper arm (rotate sites weekly) 
  • Use: This dose is typically used for the first 4 weeks of therapy before increasing to 5 mg, and further as needed based on response and tolerability. 

Storage Instructions:

  • Store in the refrigerator at 36°F to 46°F (2°C to 8°C) 
  • May be stored at room temperature (up to 86°F or 30°C) for up to 21 days 
  • Do not freeze or expose to direct sunlight or heat 

Important Safety Information:
Zepbound is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). It is not indicated for use in patients with type 1 diabetes or for the treatment of diabetic ketoacidosis.Common side effects include nausea, diarrhea, vomiting, constipation, and abdominal discomfort, especially during the initial weeks of treatment. Rare but serious risks include pancreatitis, gallbladder problems, and kidney injury. Use caution when combined with insulin or sulfonylureas due to the risk of hypoglycemia.

Clinical Use:
The 2.5 mg dose serves as the initiation step in Zepbound’s dose escalation protocol. It is not intended as a maintenance dose but rather a starting point to help patients tolerate the medication before increasing to higher doses that deliver full therapeutic benefit.
