Ozempic 4 mg/3 mL (1.34 mg/mL) prefilled pen that delivers 1 mg per injection

Ozempic® (semaglutide) is a GLP-1 receptor agonist indicated to improve glycemic control in adults with type 2 diabetes, alongside diet and exercise. It is also known for its potential benefits in weight management, though it is not FDA-approved solely for weight loss.

The 4 mg/3 mL pen (1.34 mg/mL) is a multi-dose pen that delivers 0.5 mg per injection. It is commonly prescribed as a maintenance dose after the initial 0.25 mg titration period.

How It Works:
Semaglutide mimics the action of GLP-1 (glucagon-like peptide-1), a hormone that increases insulin secretion, lowers glucagon secretion, slows gastric emptying, and reduces appetite. This leads to improved blood sugar control and may contribute to weight reduction.

Dosing and Administration:
Each prefilled pen contains 4 mg of semaglutide in 3 mL of solution (1.34 mg/mL) and is designed to deliver four weekly doses of 0.5 mg each. The pen is used once weekly via subcutaneous injection in the abdomen, thigh, or upper arm. It is important to inject on the same day each week, with or without food.

Storage Instructions:
Unopened pens should be stored in the refrigerator (36°F to 46°F or 2°C to 8°C). After the first use, the pen can be kept at room temperature (up to 86°F or 30°C) or refrigerated for up to 56 days. Avoid freezing the pen or exposing it to direct heat or sunlight.

Important Safety Information:
Ozempic is not recommended for patients with a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). It is not for use in people with type 1 diabetes or for the treatment of diabetic ketoacidosis.
Common side effects include nausea, vomiting, diarrhea, stomach pain, and decreased appetite. Rare but serious risks include pancreatitis, kidney problems, gallbladder disease, and possible thyroid tumors.

Clinical Considerations:
The 0.5 mg dose delivered from this pen is often used as the first therapeutic dose after starting with the 0.25 mg titration. Depending on the patient’s response and goals, therapy may be escalated to 1 mg or 2 mg once weekly using higher-strength pens.
